In Short

Oculoplasty is plastic and reconstructive surgery of the structures around the eye — the eyelids, the tear drainage system, the eye socket (orbit) and the bones behind it. It is performed by an ophthalmologist with additional fellowship training, because millimetres here decide both how you see and how you look.

Half of what we do restores function — lifting a lid off the pupil, unblocking a tear duct, rebuilding after trauma or a tumour. The other half is aesthetic — removing hooded skin, eye bags and dark hollows. Both are done in the same NABH-accredited theatre, usually as day care.

The lid margin sits low and shades the pupil, so you tilt your head or lift your brow to see. We measure levator function and tighten or reposition the muscle so the lid clears the pupil and matches the other side.

Hooded eyelid skin

Excess skin over the upper lid makes the eyes look heavy and can narrow your field of view. Surplus skin and a strip of muscle are removed through the natural crease, so the healed line hides where it folds.

What Happens at Your First Visit

Eyelid surgery is measured surgery. Before anything is planned we record lid height and crease position, levator muscle function, tear film and corneal health, and — where relevant — how the lids sit against your own facial proportions.

For watering eyes we syringe the tear passage to confirm where the blockage is. For orbital problems we correlate the examination with a CT or MRI scan. You leave the first visit knowing which procedure is being recommended, why, and what it will cost.

Most eyelid procedures: 30–60 minutes

Reporting & Marking

You report fasting as advised. The surgeon marks the eyelid with you sitting upright — lid position changes when you lie down, so this is done before, not on the table.

Anaesthesia

Most procedures use local anaesthesia with light sedation, so you are comfortable and awake enough to open and close the eye when asked. Children and orbital work use general anaesthesia.

The Surgery

Thirty to ninety minutes for most eyelid and tear duct procedures. Incisions follow the natural crease, lash line or the inside of the lid wherever the anatomy allows.

Check & Adjust

For ptosis and lid surgery the height and contour are checked against the other side during the procedure and adjusted before closing — the reason local anaesthesia is preferred.

Home the Same Day

Two to three hours of observation, cold compresses, drops and a written aftercare sheet. You go home the same day and are reviewed the next morning.

Recovery After Eyelid and Orbital Surgery

First 48 hours
  • Cold compresses ten minutes an hour while awake — this does more for swelling than anything else.
  • Sleep with two pillows so the head stays above the heart.
  • Antibiotic and lubricating drops or ointment exactly as prescribed.
  • Expect bruising to look worse on day two or three before it improves.
Week 1
  • Reviewed the day after surgery and again at the end of the week.
  • Sutures removed around day five to seven where non-absorbable stitches were used.
  • Desk work and screens are fine from around day five; avoid dusty sites and gyms.
  • Keep the area dry while washing; no eye make-up yet.
Weeks 2–4
  • Most visible bruising has gone; mild swelling can persist, often more on one side.
  • Make-up can usually resume once the wound is fully closed — confirmed at your review.
  • Gym, swimming and heavy lifting resume from around three to four weeks.
  • Sun protection helps the scar fade evenly.
Months 2–6
  • Scars continue to pale and soften; final contour settles by around three months.
  • Small asymmetries are reassessed at this stage, before considering any minor revision.
  • Botox is repeated at three to four months if you choose to maintain it.

Oculoplasty, Answered

The questions patients ask us most before eyelid, tear duct and orbital surgery.

Both. Oculoplasty covers functional surgery — ptosis, watering eyes, entropion, orbital injury, eyelid tumours — and aesthetic work such as blepharoplasty, Botox and tear-trough fillers. Many patients come for one and benefit from the other, because the same surgery restores both the function and the natural shape of the eyelid.

Incisions are placed in the natural eyelid crease, along the lash line or inside the lid so that healed scars are very difficult to see. Eyelid skin heals better than almost anywhere else on the body. Early redness settles over a few weeks and continues to fade for several months.

Functional procedures — ptosis affecting vision, DCR for a blocked tear duct, entropion or ectropion repair, eyelid tumour removal, orbital fracture repair and socket surgery — are usually covered and can often be done cashless. Purely cosmetic procedures such as aesthetic blepharoplasty, Botox and fillers are not covered. Our insurance desk confirms your position before the date.

Most eyelid procedures are day care under local anaesthesia and you go home the same day. Swelling and bruising peak at two to three days and largely settle within one to two weeks. Most people return to desk work in about a week, and to the gym after three to four weeks.

The surgery is done under local anaesthesia with sedation, so you feel pressure rather than pain. Afterwards there is mild soreness for a few days, managed with simple painkillers, cold compresses and the drops we prescribe.

Constant watering is often a blocked tear drainage duct, but it can also come from dry eye, an in-turned eyelid or lid laxity. A syringing and probing test at the clinic tells us which it is in minutes, and only a true blockage needs DCR surgery.

If the droop covers the pupil it can cause lazy eye and needs early surgery, sometimes within the first years of life. If vision is not blocked, surgery is usually planned around three to five years of age when eyelid measurements are more reliable. A paediatric assessment decides the timing.

Indicative ranges run from around ₹8,000 for a small lid lesion to ₹2,50,000 for orbital decompression, depending on the procedure, the anaesthesia and whether one or both eyes are treated. Your exact estimate is given in writing after examination, and Easy EMI options are available.
